To enter our drawing to win either the $150 OSU Bookstore gift certificate or one of four OSU logo items, please complete this entry form. We will be contacting the 5 winners via email or phone.

To be eligible to enter this prize drawing, you must complete the OSU 2007 Ecampus Student Survey. In addition, you must be a student who is currently taking or has taken OSU Ecampus courses. There is only one entry per student.
