The Graduate School provides many different types of resources to make your time at OSU easier. Below you will find a link to the Graduate School Forms page. Many of these forms will become more important to you as your graduate plan is created and further you get into your program. Take a moment to become familiar with these forms.

Key forms to become familiar with are:

• Change of Degree/Major
• Leave of Absence (should you decide to take a term off (excluding summer))
• Program of Study (your contract with the university for the courses needed to complete for your program)
• Petition for Change in Program (process to change the Program of Study)
• Remote Participation Form
• Transfer of Credit Request (in case you have done work at other universities)
